In the event of a confidentiality incident: we shall take reasonable measures to reduce the risk of harm to those concerned, and put measures in place to further prevent the occurrence of such incidents; we shall notify the Access to Information Commission (IAC), and any person concerned; keep a record os such incidents, a copy which must be sent ti the IAC.
